We have extended the due date to April 27th (disregard the April 15th date shown on the application).

We have two versions of the application. You can either fill it out in word online and submit it by e-mail. Or you can download it as a PDF and submit it by mail, fax, or scan and e-mail.

The applications will have three pieces. One filled in by the superintendent, one by the principal of the 7th grade teacher, and then one application from all the 7th grade math teachers per schools. We are initially looking to find enthusiastic teachers who are most likely to use ASSISTments to its full potential.
